Users must sign a new form each school year. Please print and fill out the form, then bring it with you when to check out a laptop. Leave the area for staff initials blank. Forms are also available in the Multimedia Center.

Note: If headphones are 24 hours overdue, they will be declared lost, and the patron will be responsible for the $50.00 replacement costs, a $25 processing fee, and $20.00 late fee.
